New Patients / Patient Registration Form
We require all new patients who attend our practice to complete a Patient Registration Form. For your convenience please download this New Patient registration form. This can be completed prior to coming to your first appointment with us. On arrival to your appointment, please hand it to reception staff. Please phone for an appointment on 95831630.
Missed Appointments
If you fail to attend for a booked appointment a cancellation fee may be charged. This fee is not rebatable by Medicare. It is requested that at least four hours’ notice is provided when cancelling or changing an appointment to allow for appropriate scheduling of appointments.
Updating your records
It is very important that we are able to contact you. Please let us know of any change of address, work, home or mobile number, Medicare number, a person to contact in case of an emergency, or a change of name. Please contact the practice to update these details.
After Hours
The Medical Clinic provides out-of-hours care via National Home Doctor Service.
Please call 13 SICK (13 7425) or visit their website www.homedoctor.com.au for services and fees.
All medical notes relating to a visit from National Home Doctor Service is downloaded electronically directly into your medical history.
If your matter is urgent, please present to your local hospital emergency department or call emergency TRIPLE ZERO (000).

Results
Test results are generally available within 2-3 days of the test being performed. Some tests can take considerably longer, but most are electronically download to our doctors by our preferred pathology and imaging providers.
To ensure confidentiality and best care, we prefer that you make a follow up appointment to discuss test results. These discussions allow you a better understanding of your medical care. For urgent test results we will best endeavour to notify you as soon as is practically possible, but please call if you have not heard about your result.
Home Visits
Urgent home visits within normal opening hours are available if a patient is too ill to attend, and where it is safe, reasonable and possible to do so. The patient must be a regular patient of the practice and live nearby to the medical centre. All home visits will attract a private fee and be booked at the sole discretion of the doctor.
Telephone and Electronic Communication
Our doctors may be contacted by phone during normal consultation hours through our reception team. Communication via email is kept to a minimum due to health information privacy and confidentiality requirements.
Minor Procedures
The costs of minor medical or surgical procedures vary on the type of procedure and its complexity. However prior to any planned procedures we aim to provide information about any anticipated costs.
Vaccines & Immunisations
Childhood vaccinations and immunisations are free if you are a Medicare card holder and meet DHS criteria. Our comprehensive range of travel vaccinations attracts an additional private charge and the cost may be claimable by your Private Health Insurance by contacting them directly.
Scripts & Referrals
All NEW specialist referrals and medications will always require a REGULAR appointment with your doctor.
The general policy of the medical clinic is that all repeat prescriptions and ongoing specialist referrals require a SHORT appointment with your doctor. These appointments allow your doctor to review the condition being treated; ensure your medical information is current and accurate, and also serve as an opportunity to plan for other health issues.
GP Registrar
We welcome new GP registrars to the Bayside Family Medical team every twelve months or so. You are able to get an appointment with them as you would with any of our medical team.
A GP Registrar is a fully qualified medical doctor, undertaking advanced specialist training in General Practice. The Royal Australian College of General Practitioners requires that doctors complete an advanced GP training program. Over at least two years, GP Registrars will attend placements at up to three different medical practices to advance their skills.
Where to find us
Our main reception is located on Nepean Highway but we are also easily accessed from Charman Road.
We are located near the Cheltenham train station and are readily accessible by bus services.
There is car parking onsite and also adjacent to the clinic.

Your satisfaction is important to us. If you feel you have an idea of areas where we can improve, please complete our patient feedback form and place it in the suggestion box which is located at reception. If you have a complaint, please discuss this with your Doctor or our Practice Manager.
